Module 2: Methods of Medication Dosage Calculation

    Basic Formula

    Clindamycin 0.5 g, per oral (po) every 9q) 8 h

    Since the units of measurement are different, a conversion is necessary

    First, perform the ratio & proportion method of conversion to change grams to milligrams.

    1g : 1000mg : : 0.5 g : X mg
    1000 x 0.5 = 500
    X = 500 mg

    Second, (D / H) x V = (500 / 250) X 1 tab = 2 tabs Clindamycin
